It was once the domain of movie stars or the wealthy, but today many forms of cosmetic surgery are within reach of a much larger cross-section of the community.

And with the latest advances in surgical techniques, including the use of special lasers, this type of surgery has become a lot safer and has generated interest among women and men of all ages.

In response to the incredible growth and public interest in this field, New Idea is launching this exciting new Body Works column with Doctor Daniel Lanzer, a qualified specialist dermatologist with a long-standing interest in cosmetic surgery.

He is one of the founding Fellows of the Australian Division of the International Society of Cosmetic Laser Surgeons and keeps pace with the ever-changing world of cosmetic laser surgery by attending various international seminars and lectures throughout the year.

Each week he will be discussing the latest cosmetic laser surgery techniques and providing plenty of advice to anyone who has any questions about cosmetic surgery.

The cosmetic surgery techniques that particularly interest Dr Lanzer include: tumescent liposuction, which is seen as one of the safest forms of fat removal; laser resurfacing of the skin, particularly the face; and laser eyelift surgery. The great benefit of these revolutionary forms of surgery is that they are virtually pain-free.

SMOOTH OUT THOSE WRINKLES!

At the time when laser surgery was just commencing in Australia Ray Martin asked me on The Midday Show when was the right stage in the ageing process for laser surgery, as opposed to facelift scalpel surgery. This is an important question and one crucial to the understanding of what type of surgery is needed by a person who is considering cosmetic rejuvenation.

With ageing and sun damage to the skin, there are two distinct phases. Initially, the skin develops blotchy pigmentation, loss of elasticity, fine wrinkling and, eventually, deeper furrows.

The second state occurs when the skin and muscle of the face actually start to fall and hang from the bone structures. It is the first phase that responds to laser ultrapulse skin resurfacing. And it is the second phase that requires facelift scalpel surgery.

I have seen many patients over the years who are unhappy following their facelift surgery because they had not fully considered whether they were after a tighter, smoother skin for resurfacing, or a correction of sagging.

Laser resurfacing is particularly helpful for the wrinkles around the mouth and around the eyes – see the before and after pictures above and right for an idea of what this technique can do. The procedure is usually performed under a general anaesthetic and healing occurs in 7-10 days. At that point the skin is usually pink, but this can be covered with makeup and the patient can carry on as normal.

LASER SURGERY WITH EYE-OPENING RESULTS

One of the most satisfying operations in cosmetic surgery is the removal of ‘bags’ under the eyes.

Some patients develop fatty deposits which occur either as part of a genetic tendency or simply with ageing.

The bags tend to give a tired appearance to the eyes and face, which is caused mainly by the shadow they cast to the area below them.

The CO2 Laser has dramatically changed the way we can correct these puffy bags of fat.

Rather than cut the layer of skin on the outside and suture the skin together, our procedure sees the removal of all the fat from the inside of the lower eyelid without any visible cuts or scars.

Using the laser in this procedure has the added advantage of sealing the blood vessels while it cuts.
To the patient, this procedure means, less bruising, swelling and no bleeding.

LIPOSUCTION HELPS REGAIN YOUR WAISTLINE


Liposuction has been with us for quite a few years, and now there is a new liposuction technique, tumescent liposuction, which has been tried on more than 15,000 patients in the United States.

This new technique, developed by dermatologists, is safer and more effective (see before and after shots). Basically, this procedure involves filling up the fat with a very dilute local anaesthetic liquid, before sucking up fat through a small nick in the skin.

The liquid numbs the fat, which means that there is less or no pain after the operation.

The liquid also closes up the little blood vessels so that there is no bleeding and never a requirement for a blood transfusion or intravenous fluid.

Also, the liquid is anti-bacterial and this almost totally removes the risk of infection.

After surgery, there is slight tenderness of the treated area but patients can usually return to work a few days after the procedure.

The approximate cost of liposuction is $3000. An extra $700 will be charged for other individual regions.

MOVE THAT BUTT! DOWNSIZE YOUR SEAT WITH LIPOSUCTION

Probably the most common areas on which liposuction is performed are the inner thighs and buttocks. It is in these areas that patients sometimes find it hard to determine exactly where they need fat removed. One interesting test I suggest to patients is to contract or tighten up their buttocks and have a look at the fat on the outer thighs. If the fat disappears into the bottom, then it is a sign the patient could have the fat removed from the outer bottom as well as the outer thighs.

The tumescent liposuction technique includes infiltrating into the skin an extremely dilute local anaesthetic solution. This liquid numbs and softens the fact and allows it to be sucked out using very fine, long needle-like tubes that are attached to a suction machine. Using this technique, large amounts of fat can be removed in one operation. This procedure can be performed while the patient is awake or under a general anaesthetic. Some patients actually prefer just to have light sedation.

We recommend that patients wear a girdle for the following two weeks, which helps the fat and skin retract inwards.

Liposuction for the bottom costs $3000 plus $700 for each additional area.

MEN & COSMETIC SURGERY

Over the past 10 years, an increasing number of men are choosing to undergo various cosmetic surgery procedures. In the U.S., about one-third of cosmetic surgery is performed on males. Although in Australia cosmetic surgery is still mostly performed on females, similar trends are being seen here.

Males are taking advantage of the newer, safer techniques of liposuction. As opposed to women, where the most common areas of concern are the outer thighs and buttocks, in males most liposuction is performed on the abdomen or outer flanks. Another common area for liposuction is the breasts. The occurrence of large breasts in males is called pseudo gynecomastia and is a collection of fat under the breast tissue.

Liposuction is performed through a small nick in the skin and the fat is initially filled up with an extremely diluted local anaesthetic solution. The solution numbs and softens the fat, which is then sucked out through a small tube attached to a vacuum pump. Usually, many areas can be treated on one occasion and patients are back to work within the week.

With the tumescent technique, there is far less bruising and a girdle or compression garment is generally worn for two weeks following surgery.

Hair transplants have been performed for many years and the newer techniques involving small slit-like incisions as opposed to the rounder punch-grafts have become popular. The result is a more natural appearance following the transplant.
